Mounjaro Results: How Much Weight Can You Lose in 1 Month, 3 Months and a Year?

In the SURMOUNT-1 trial, adults without diabetes lost an average of 15% to 21% of body weight over 72 weeks, depending on dose. Month 1 is usually modest because 2.5 mg is a starting dose, and most of the loss builds over months, not weeks.

In the main Mounjaro (tirzepatide) weight-loss trial, adults without diabetes lost an average of 15.0% of body weight at 5 mg, 19.5% at 10 mg and 20.9% at 15 mg over 72 weeks, against 3.1% on placebo. That is the honest headline. The first month is usually much smaller, because you start on 2.5 mg, a dose meant to get your body used to the medicine rather than to drive weight loss.

This guide walks through what the trials actually showed over time, why early results vary so much, and what you can control. It uses trial averages only. Your own results will differ, and nobody can promise you a number of kilos.

Key takeaways

  • SURMOUNT-1 (72 weeks, no diabetes): average loss of 15.0% at 5 mg, 19.5% at 10 mg, 20.9% at 15 mg, vs 3.1% on placebo.
  • Month 1 on 2.5 mg is an adjustment dose. A small change on the scale is normal and says little about where you will end up.
  • Weight loss builds over months as the dose steps up. Most of the trial loss came well after the first 12 weeks.
  • Dose, protein intake, strength training and sleep shape your results as much as the pen does.
  • Results vary widely between people. Trial numbers are averages, not a guarantee.

What the trials showed at 72 weeks

SURMOUNT-1 enrolled adults with obesity, or overweight with a weight-related condition, who did not have diabetes. Everyone also got advice on a reduced-calorie diet and more activity. After 72 weeks, the average results were:

15.0%
Average weight loss at 5 mg (72 weeks)
19.5%
Average weight loss at 10 mg (72 weeks)
20.9%
Average weight loss at 15 mg (72 weeks)
3.1%
Placebo, same diet and activity advice

In practical terms, for someone starting at 95 kg, 15% is about 14 kg and 20% is about 19 kg. Those are averages over roughly a year and a half, on a steady maintenance dose. Some people in the trial lost much more, some lost much less.

A later head-to-head trial, SURMOUNT-5, compared tirzepatide with semaglutide over 72 weeks: 20.2% average loss on tirzepatide against 13.7% on semaglutide. If you are weighing the two, see semaglutide vs tirzepatide in India.

Mounjaro weight loss in 1 month: why it is small

Every patient starts on 2.5 mg once a week for 4 weeks. This is the adjustment dose. Its job is to let your stomach and gut get used to the medicine so side effects stay manageable when the dose goes up. It is not the dose that produces trial-level weight loss.

In SURMOUNT-1 the dose went up by 2.5 mg every 4 weeks, so people assigned to 15 mg only reached it at week 20. The 72-week results reflect about a year on a full maintenance dose. Comparing your first month on 2.5 mg with the 72-week headline is like judging a new job by your first week of training.

What most people notice in month 1 is appetite, not the scale: smaller portions feel enough, snacking drops, and cravings quieten, especially in the first few days after each dose. Some of the early drop on the scale is water and gut content, so do not read too much into it either way. For a week-by-week view of the start, read your first month on Mounjaro, week by week.

A slow start is not a failed start

A post hoc analysis of SURMOUNT-1 presented at AACE 2024 looked at people who had lost less than 5% by week 12. About 90% of them still went on to lose at least 5% by week 72. A small early result is common and usually not a reason to give up.

How much weight can you lose on Mounjaro in 3 months?

There is no honest single figure, and you should be wary of anyone who gives you one. The trials did not headline a 12-week average, and the usual Indian 3-month ladder (2.5 mg, then 5 mg, then 7.5 mg) is still below the doses most trial participants were on for most of the study.

What the trials do show is the shape of the curve: loss starts gradually, speeds up as the dose climbs through months 2 to 5, and keeps going well into the second half of the study before levelling off. In SURMOUNT-4, where everyone took tirzepatide at their maximum tolerated dose (10 or 15 mg) for 36 weeks before randomisation, the average loss at week 36 was about 21%. That is nine months, not three.

So think of the first three months as the phase where you find your dose, settle your side effects and build habits that make the later months count. The tirzepatide dose chart shows how the steps usually work.

What actually drives your Mounjaro results

1. Dose, and staying on it

In SURMOUNT-1, higher doses produced more weight loss on average. Your doctor steps you up by 2.5 mg no sooner than every 4 weeks, up to a maximum of 15 mg, based on how you respond and how well you tolerate it. Missing doses or stopping early is one of the most common reasons results stall.

2. Protein at every meal

When you eat less, some of what you lose can be muscle. Aim for a protein source in every meal: dal, paneer, curd, eggs, chicken, fish, soya, sprouts. Vegetarians can get there with planning, and a nutritionist can help.

3. Strength training

Two or three sessions a week of resistance exercise (weights, bands, bodyweight squats and push-ups) helps protect muscle, which keeps your metabolism and strength up as the kilos come off. Walking is good; walking plus strength is better. More in muscle loss on GLP-1 and how to prevent it.

4. Sleep and routine

Short sleep raises hunger and makes late-night eating more likely. A regular sleep window, a fixed injection day and simple meal routines make the medicine’s effect on appetite easier to use.

5. Your starting point

Starting weight, age, sex, other conditions (like type 2 diabetes, where average weight loss in trials was lower) and other medicines all affect how much you lose. This is why a doctor, not a chart, should set your expectations.

About Mounjaro before and after photos

Before-and-after photos online are selected, not representative. You rarely know the dose, the time frame, the diet or whether the photos are even real. We do not publish patient photos or testimonials. The trial averages above are a better guide to what is realistic, and your doctor can tell you what is realistic for you.

Track more than the scale

Weigh yourself once a week on the same morning, and also note your waist measurement, how your clothes fit, your energy and your blood reports. The scale bounces day to day; the trend over 4 to 8 weeks is what matters.

What happens if you stop

In SURMOUNT-4, people who switched to placebo after 36 weeks regained a large share of the weight over the following year, while those who continued kept losing. Mounjaro manages weight while you take it; it does not cure the tendency to regain. Plan for maintenance with your doctor, see what happens when you stop a GLP-1.

How ALTRcare supports your first 3 months

Our tirzepatide 3-month plan (Standard Care) is ₹34,999, or ₹11,666 a month. Frequently asked questions

How much weight can you lose on Mounjaro in 1 month?

Usually not much, and that is expected. Month 1 is spent on 2.5 mg, an adjustment dose meant to let your body get used to the medicine. Most weight loss in the trials came over the following months as the dose increased.

How much weight can you lose on Mounjaro in 3 months?

It varies too much between people to give one honest number. The trials reported their main results at 72 weeks, and the usual 3-month ladder of 2.5, 5 and 7.5 mg is below most trial maintenance doses. Your doctor can set realistic expectations for you.

What were the Mounjaro results in clinical trials?

In SURMOUNT-1, adults without diabetes lost an average of 15.0% (5 mg), 19.5% (10 mg) and 20.9% (15 mg) of body weight over 72 weeks, against 3.1% on placebo. Results varied between individuals.

Why am I not losing weight on Mounjaro 2.5 mg?

2.5 mg is a starting dose, not a weight-loss dose. A small or slow change in the first month is common. If progress is still slow after dose increases, talk to your doctor about dose, diet and protein.

Is Mounjaro more effective than semaglutide?

In the SURMOUNT-5 head-to-head trial over 72 weeks, average weight loss was 20.2% on tirzepatide and 13.7% on semaglutide. Which one suits you depends on your health, tolerance and budget.

Do you regain weight after stopping Mounjaro?

Many people do. In SURMOUNT-4, people switched to placebo regained a large share of the weight they had lost. Plan maintenance with your doctor before stopping.
