Mounjaro First Month: What to Expect Week by Week on 2.5 mg

Your first month on Mounjaro is 2.5 mg once a week for 4 weeks. Expect a quieter appetite, possibly mild nausea in the first few days after each dose, and only a modest change on the scale. In month 2 your doctor usually steps you up to 5 mg.

In your first month on Mounjaro you take 2.5 mg once a week, on the same day each week, for 4 weeks. Most people notice their appetite first: smaller portions feel enough and snacking fades, strongest in the first few days after each injection. Weight change in month 1 is usually modest, and that is normal, because 2.5 mg is a starting dose, not a weight-loss dose.

Key takeaways

  • Month 1 = 2.5 mg weekly for 4 weeks. It is an adjustment dose to let your gut get used to the medicine.
  • Appetite suppression is usually strongest on days 1 to 3 after the dose and eases on days 4 to 7. Hunger returning before the next dose is normal.
  • Mild nausea, burping, constipation or loose stools are common in weeks 1 and 2 and usually settle.
  • Pick a fixed injection day, eat light that evening and the next day, and keep fluids up.
  • From month 2 your doctor usually moves you to 5 mg, and you may feel week 1 symptoms again briefly.

Before week 1: getting set up

  • Choose an injection day you can stick to, such as every Sunday morning. You can inject at any time of day, with or without food.
  • Store the pen in the fridge (2–8°C). Do not freeze it. Learn the steps in how to use the Mounjaro KwikPen.
  • Note your starting weight, waist measurement and a photo for yourself, so you can track the trend later.
  • Stock the kitchen with easy meals: dal, curd, paneer, eggs, khichdi ingredients, fruit, and chaas or buttermilk.
  • If you take insulin or a sulfonylurea for diabetes, or oral contraceptive pills, tell your doctor before the first dose.

The weekly appetite curve

Mounjaro is a once-weekly medicine, and many people feel a pattern across each week:

How a typical Mounjaro week can feel
Days after the doseWhat many people notice
Day 1 to 3Strongest appetite effect. Feel full quickly. Nausea, if any, is most likely now.
Day 4 to 5Effect eases a little. Meals feel more normal, cravings still quieter than before.
Day 6 to 7Hunger may return, especially in the evening. This is normal and not a sign the medicine has stopped working.
How a typical Mounjaro week can feel · Patterns vary from person to person. On 2.5 mg the returning hunger near the end of the week is more noticeable than on higher doses.

Hungry before the next dose? That is expected

Do not take the next dose early or add an extra dose. Plan protein-rich meals and filling snacks (roasted chana, curd, sprouts, fruit) for days 6 and 7, and mention it to your doctor at your dose-review consultation.

Week 1: the first injection

The injection itself is quick, a small click and about 10 seconds of holding the pen in place. Some people feel a change in appetite within a day or two; others feel very little at 2.5 mg. Both are normal.

The most common early effects are mild nausea, burping, a heavy stomach after a normal-sized meal, and changes in bowel habit. They are usually mild. Eat smaller meals, stop when you first feel full, and avoid heavy fried food for the first 48 hours after the injection.

Week 2: settling in

Your second dose often feels easier than the first as your body adjusts. This is a good week to build the habits that matter later: a protein source at every meal, 2 to 3 litres of fluid a day, and a daily walk. If constipation starts, add water, vegetables and fruit before anything else.

Week 3: noticing the pattern

By now you will recognise your own weekly curve. Use it: keep social dinners or heavier meals for later in the week when appetite has eased, and keep the day of the dose and the day after simple. Start two short strength sessions a week (squats, wall push-ups, resistance bands) to protect muscle.

Week 4: dose review

Your fourth 2.5 mg dose finishes the pen. Around now your doctor reviews how you are doing: side effects, appetite, weight trend, and any other health changes. If you are tolerating it, the usual next step is 5 mg. If side effects were difficult, the doctor may keep you on 2.5 mg a little longer. Doses go up by 2.5 mg no sooner than every 4 weeks, to a maximum of 15 mg. The full ladder is in the tirzepatide dose chart.

What about the scale in month 1?

A small change is normal on 2.5 mg, and some of it is water. Weigh once a week on the same morning and look at the trend over 6 to 8 weeks. For what the trials showed over longer periods, read Mounjaro results.

Injection day tips

  • Take the pen out of the fridge about 30 minutes before, so it is not cold when you inject.
  • Rotate sites: belly (at least 5 cm from the navel), front of the thigh, or back of the upper arm if someone helps you.
  • Eat a light dinner that evening: khichdi, dal-chawal, curd rice, soup.
  • Keep a glass of chaas, thin lassi or water nearby and sip through the next day.
  • Missed a dose? If it is within 4 days (96 hours), take it when you remember. If more than 4 days have passed, skip it and take the next one on your usual day. Never take two doses within 3 days of each other.

Month 2: what changes at 5 mg

5 mg is the first dose that was studied as a maintenance dose in the trials. At this step many people feel a stronger, longer-lasting appetite effect across the week, and the return of hunger on days 6 and 7 is often less noticeable.

Side effects can come back for the first week or two after the step up, often similar to your first week on 2.5 mg, and then settle again. Use the same approach: small meals, light food on dose day, plenty of fluids. If you take oral contraceptive pills, use a backup method for 4 weeks after this dose increase too.
