How to Use the Mounjaro or Yurpeak KwikPen: Step by Step, With the Mistakes to Avoid

The tirzepatide KwikPen holds four weekly doses and has no dose dial to set. Here is the full routine from fridge to sharps bin, the eight mistakes we see most in the first month, and what to do if something goes wrong mid-injection.

The KwikPen is the multi-dose device that Mounjaro and Yurpeak come in for India: one pen, four weekly doses of a single strength, a fresh needle each time. It is simpler than the Ozempic-style pen because there is no dose to dial, but that simplicity hides the two things people get wrong: not priming, and not holding the pen in place long enough. This is the routine our doctors walk every patient through on the first injection call, written down.

Mounjaro vials are different

Mounjaro was first launched in India in single-dose vials, drawn up with a syringe. Those instructions are not the same as the pen. This guide is for the KwikPen. If you have vials, your doctor will show you the syringe method.

What comes in the box

  • One KwikPen of a single strength (2.5, 5, 7.5, 10, 12.5 or 15 mg per dose), containing four doses.
  • Needles are usually sold separately in India. You need 4 mm or 5 mm pen needles, 32G is standard. Ask the pharmacy or your ALTRcare delivery for the right ones.
  • The leaflet. Keep it; the batch number on the box is what you quote if anything seems off.

Before your first dose

  1. 1Take the pen out of the fridge 15 to 30 minutes before injecting. Cold tirzepatide stings more. Do not warm it under a tap or in the sun.
  2. 2Wash your hands. Check the label says the strength your doctor prescribed and the expiry date has not passed.
  3. 3Look at the liquid through the window. It should be clear and colourless. Cloudy, coloured or with particles means do not use it.
  4. 4Pick your site: abdomen at least 5 cm from the navel, front of the thigh, or the back of the upper arm if someone else is injecting you. Rotate sites every week.

The injection, step by step

  1. 1Attach a new needle. Pull the paper tab off, screw the needle straight onto the pen until snug. Remove the outer cap, keep it, then remove the inner cap and discard it.
  2. 2Prime the pen, every time, on a new pen only. Turn the dose knob until it stops. Hold the pen with the needle pointing up, tap the cartridge so bubbles rise, and press the injection button in fully. You should see a drop or a thin stream at the needle tip. If nothing appears, repeat up to four times. If still nothing, the pen or needle is faulty; do not inject.
  3. 3Turn the dose knob until it stops. This sets the full dose. On the KwikPen you cannot set less; that is by design.
  4. 4Clean the site with an alcohol swab and let it dry for a few seconds.
  5. 5Insert the needle straight in, at 90 degrees. You do not need to pinch the skin with a 4 mm needle unless you are very lean.
  6. 6Press the button all the way down and hold for a slow count of five. This is the step people rush. The dose is still going in during the hold. Let go early and part of it stays in the needle.
  7. 7Withdraw, then check the dose window shows 0. If it does not, the full dose was not delivered; do not inject again, note the number and message your doctor.
  8. 8Remove the needle using the outer cap, and drop it in a sharps container or a hard plastic bottle with a lid. Never reuse a needle; it blunts after one use and blocks the pen.

What goes wrong, and what it costs

Storage in an Indian summer

Unopened pens live in the fridge at 2 to 8°C. Once you start a pen, it can be kept at room temperature up to 30°C for up to 21 days, which covers its four weekly doses in most cases, but in April to June in most of India room temperature is above 30, so keep it in the fridge between doses. Do not freeze it; a pen that has frozen is finished even after thawing. For travel, an insulated pouch with a gel pack keeps it in range for 8 to 10 hours; put it in your cabin bag, never checked luggage.

If something goes wrong

  • Liquid leaked from the site: a drop or two is normal. If it ran down your skin, you likely withdrew early. Do not re-inject; take your next dose on schedule and mention it to your doctor.
  • Blood at the site: press with a swab for 30 seconds. Small bruises are common and harmless.
  • Dose window did not return to 0: the dose was incomplete. Note the number shown and message your doctor before doing anything else.
  • You missed the day: inject as soon as you remember if it is within 4 days of the scheduled dose. If more than 4 days, skip it and take the next one on the usual day. Never take two doses within 3 days of each other.
  • You cannot get a drop when priming: try a fresh needle. If still nothing, the pen is faulty. Keep it and the box for your pharmacy or ALTRcare to replace.
