Why the monthly bill goes up
Tirzepatide is always started low. You take 2.5 mg a week for four weeks, then 5 mg. After that your doctor may step up by 2.5 mg no sooner than every four weeks, up to 15 mg, depending on how you respond and how you tolerate it. The dose steps are laid out in our tirzepatide dose chart.
The 2.5 mg month exists to let your stomach adjust. The weight loss seen in trials came from 5 mg and above: an average of 15.0% of body weight at 5 mg and 20.9% at 15 mg over 72 weeks in the SURMOUNT-1 trial. So budget for the dose that works, not the starter pen.

Is Mounjaro worth it over semaglutide?
In the SURMOUNT-5 head-to-head trial, tirzepatide produced 20.2% average weight loss against 13.7% for semaglutide over 72 weeks.
