A weekly calendar with an injection day marked and a light Indian meal beside it

The short version: on the day you take your weekly GLP-1 dose, eat lighter than usual, do not "stock up" with a big meal beforehand, keep sipping fluids through the day, and take the injection at roughly the same time each week. The medicine's effect follows a weekly curve: it peaks on days 1 to 3 after the shot, then tapers on days 4 to 7. Once you understand that curve, almost everything about dose day makes sense.

First, understand the weekly curve

Semaglutide and tirzepatide are once-weekly injections, but their effect is not flat across the week. Appetite suppression is strongest in the first two to three days after your dose, when drug levels are near their peak. By days 5 to 7, levels have tapered and many patients notice their appetite creeping back before the next dose.

Returning hunger before your next dose is normal

If you feel hungrier on day 6 than on day 2, the medicine is not "wearing off" or failing. That is the expected shape of a weekly drug. It is also not a signal to take your next dose early: stick to the weekly schedule your doctor set.

What to do on the day of your dose

  1. 1Take it at a consistent time. Same day each week, roughly the same time of day. Consistency keeps drug levels steady and makes side effects more predictable. If you need to shift your dose day permanently, ask your doctor how to move it safely.
  2. 2Eat lighter, not heavier. The most common dose-day mistake is a big "last proper meal" before the shot. Food already in your stomach empties more slowly once the dose kicks in, which is exactly how nausea and heaviness start. Keep dose-day meals small and simple: dal, khichdi, curd rice, a light sabzi-roti plate.
  3. 3Sip fluids through the day. Water first. Chaas, thin lassi, coconut water and zero-sugar cold drinks are good options if plain water feels unappealing. Dehydration makes nausea, headaches and fatigue noticeably worse in the first 48 hours.
  4. 4Go easy on oil, spice and fried food. Heavy, oily or very spicy meals on dose day and the two days after are the most reliable way to trigger stomach upset. Save the indulgent meal for later in your week if you want one.
  5. 5Skip or strictly limit alcohol. Alcohol on a slowed stomach hits harder and irritates it more. If you drink at all, keep it away from the first 48 hours after your dose.
  6. 6Rotate your injection site. Stomach, thigh or upper arm, a couple of finger-widths away from last week's spot. Same body area is fine; same exact spot every week is not.

Days 1 to 3: the peak window

This is when appetite suppression is strongest and when side effects, if you get them, are most likely. Work with it rather than against it: smaller portions, protein first at each meal (dal, paneer, eggs, curd, chicken), and no pressure to finish a full plate. Many patients find these the easiest days of the week for eating discipline, so this is a good window to practise the portion sizes you want to keep long term.

Days 4 to 7: the taper

As drug levels taper, hunger returns a little. This is where habits do the work the medicine did earlier in the week: planned meals, protein at every meal, and fluids before you reach for a snack. If late-week hunger feels unmanageable rather than just noticeable, tell your doctor. It is useful information for your next dose review, not a problem to push through silently.

What NOT to do on dose day

  • Do not double up or take a dose early because you feel hungry late in the week.
  • Do not take the shot for the first time, or after a dose increase, right before a big event, travel day or wedding function. Give yourself a quiet 48 hours.
  • Do not inject a pen that has been left out of the fridge beyond its allowed window or looks cloudy or discoloured. Ask your care team first.
  • Do not skip fluids because your appetite is low. Low appetite suppresses thirst signals too.
  • Do not treat a missed dose casually. The catch-up rules depend on how many days have passed, so check with your doctor rather than guessing.

Key takeaways

  • Take your dose on the same day and rough time each week.
  • Eat lighter on dose day, never a big "stock up" meal.
  • Hydrate steadily: water, chaas, thin lassi, coconut water.
  • Effect peaks days 1 to 3 and tapers days 4 to 7; returning late-week hunger is normal.
  • Never take a dose early to fight late-week appetite. Flag it to your doctor instead.

Frequently asked questions

Should I eat before or after my GLP-1 injection?

Either is fine; the injection does not need to be timed to meals. What matters is keeping dose-day meals light. A heavy meal near your dose is the most common trigger for nausea.

Why am I hungrier at the end of the week on semaglutide?

Drug levels peak in the first 2 to 3 days after your weekly dose and taper toward day 7, so some returning appetite before your next dose is the normal shape of a weekly medicine, not a sign it has stopped working.

What should I drink on injection day?

Water is the base. Chaas, thin lassi, coconut water and zero-sugar cold drinks are good additions if plain water is unappealing. Avoid or strictly limit alcohol in the 48 hours after your dose.

Can I change my injection day?

Usually yes, but the safe way to shift depends on how many days sit between doses, so confirm the exact adjustment with your doctor instead of moving it on your own.

Is it okay to exercise on dose day?

Yes. Normal activity and workouts are fine on dose day. Just hydrate well, and if you feel queasy in the first 24 to 48 hours, a walk is a better choice than a heavy session.
