GLP-1 Eligibility Criteria in India: The Checklist Doctors Actually Use

BMI thresholds (the Indian ones, not the Western ones), the conditions that qualify you at a lower BMI, and the health checks that come before any prescription.

The short answer: in India, you are typically a candidate for GLP-1 weight-loss medication if your BMI is in the obese range on Indian cutoffs, or in the overweight range WITH at least one weight-related condition such as prediabetes, type 2 diabetes, PCOS, fatty liver, high blood pressure or sleep apnea. On top of that sits a safety screen: a short list of conditions that rule the medicines out or require caution. The final call is always a doctor's clinical judgement, not a calculator's.

Why Indian BMI cutoffs are lower

South Asians develop diabetes, fatty liver and heart disease at lower body weights than Western populations, partly because we carry more fat around the organs at the same BMI. Indian guidelines therefore set the overweight line at a BMI of 23 (not 25) and the obesity line at 25 (not 30). A BMI of 24 that would be called "normal" on a Western chart is already overweight on the chart that actually applies to Indian bodies, which is why waist size and metabolic health carry real weight in the assessment too.

The eligibility checklist

How doctors assess GLP-1 candidacy for Indian patients
Your situationAre you a candidate?
BMI 25 or higher (Indian obesity range)Yes, typically a candidate on BMI alone, subject to the safety screen
BMI 23 to 24.9 WITH a weight-related conditionYes, typically a candidate. The condition is what qualifies you
BMI 23 to 24.9 with no conditions and a normal waistUsually not first-line. Lifestyle-led care makes more sense
BMI below 23Generally not a candidate for weight-loss medication, with rare specialist exceptions
Any BMI, but pregnant, breastfeeding or trying to conceiveNo. This is an absolute exclusion

Conditions that qualify you at a lower BMI

  • Prediabetes or type 2 diabetes: the clearest case, since GLP-1 medicines treat the sugar problem and the weight together.
  • PCOS: insulin resistance drives both the weight and many PCOS symptoms, which is why weight loss is first-line treatment.
  • Fatty liver (NAFLD/MASLD): weight loss of 7 to 10 percent is the single most effective known treatment.
  • High blood pressure or abnormal cholesterol: both improve measurably with meaningful weight loss.
  • Obstructive sleep apnea: weight is the biggest modifiable driver, and tirzepatide in particular has trial evidence here.
  • Joint pain from weight (knees, back): every kilogram lost takes roughly four kilograms of load off the knees per step.

The safety screen that runs alongside

Meeting the BMI and condition criteria is necessary but not sufficient. Before prescribing, a doctor also screens for the exclusions: personal or family history of medullary thyroid cancer or MEN 2, pregnancy or plans for it, prior pancreatitis, gallbladder disease, severe stomach conditions, an eating disorder history, and medicines like insulin or sulfonylureas that need dose adjustment. If a provider skips these questions entirely, that is a provider problem, not a shortcut in your favour.

What a doctor checks before saying yes

  1. 1Your numbers: weight, height, waist, and BMI on Indian cutoffs.
  2. 2Your conditions: diagnosed or suspected. "My sugar was borderline last year" is relevant information, say it.
  3. 3Your medications and history: the full safety screen above.
  4. 4Baseline blood work where needed: commonly HbA1c, lipids, liver and kidney function, and thyroid, depending on your history.
  5. 5Your goals and timeline: a 6-plus-month treatment plan, not a crash course before one event.

Eligible for a consultation is the honest phrasing

No article, quiz or checklist can prescribe. What an eligibility check tells you is whether a doctor consultation is worth your time. The prescription decision happens inside that consultation.

Key takeaways

  • Indian cutoffs apply: overweight starts at BMI 23, obesity at 25.
  • BMI 25+ typically qualifies on its own; BMI 23 to 24.9 qualifies with a metabolic condition.
  • PCOS, fatty liver, prediabetes, hypertension and sleep apnea are the common qualifying conditions.
  • A safety screen for exclusions runs alongside the BMI check every time.
  • The checklist gets you to a doctor; only the doctor gets you to a prescription.

Frequently asked questions

What BMI qualifies for weight-loss medication in India?

On Indian cutoffs, a BMI of 25 or above typically qualifies on its own, and a BMI of 23 to 24.9 qualifies when you also have a weight-related condition like PCOS, prediabetes, fatty liver or hypertension. A doctor confirms the final decision.

Can I get a GLP-1 prescription with a normal BMI but PCOS?

Sometimes, yes. Metabolic conditions at a near-normal BMI are a legitimate clinical case on Indian cutoffs, and this is exactly the situation where a doctor's judgement, not a BMI calculator, should decide.

Do I need blood tests before starting?

Doctors commonly want baseline HbA1c, lipids, liver, kidney and thyroid values, depending on your history. Recent reports often suffice; if you have none, the tests are inexpensive and quick to arrange.

Does age affect eligibility?

You must be 18 or older for standard adult prescribing. Older adults are often still eligible, with extra attention to muscle preservation, kidney function and other medications.

Is eligibility different for semaglutide vs tirzepatide?

The core criteria are the same. Which molecule suits you better is a separate decision based on how much weight you need to lose, your budget and your doctor's judgement.
