Why the same drug has two names
Novo Nordisk ran semaglutide through two separate development programmes. The diabetes programme produced Ozempic, approved on glycaemic control data with weight loss as a secondary benefit. The obesity programme ran the STEP trials at higher doses and produced Wegovy, approved specifically for chronic weight management.

The dose ceiling is the real difference
This is the part that gets missed. In India, Ozempic is sold in three dose strengths: 0.25 mg, 0.5 mg and 1 mg. Wegovy has five, reaching the 2.4 mg maintenance dose used in the obesity trials.
If you are being treated for weight rather than blood sugar, your titration is heading towards that higher dose. Starting on Ozempic and then discovering it does not go high enough means switching brands mid-course, which is manageable but avoidable. Choosing based on where your titration ends, not where it starts, saves that.
Off-label use is common and not the same as illegal
Ozempic is widely prescribed off-label for weight loss in India and elsewhere. That is a legitimate clinical decision a doctor can make. But it is worth knowing that you are then working within a narrower dose range than the obesity trials used.

On 1 April 2026, facing the generic wave, Novo reduced the starting dose of Ozempic by 36% and Wegovy by 48%, bringing both to ₹1,415 per weekly shot — an effective ₹202 per day. Averaged across all doses the reductions were smaller: about 24% for Ozempic and 27% for Wegovy.
The headline parity is real but only at the starting dose. Because Wegovy titrates higher, your monthly cost on Wegovy rises further than on Ozempic as you move up. Budget for where you will be at month four, not month one.
The comparison most people should actually be making
Since March 2026, generic semaglutide from Alkem, Zydus, Sun Pharma and others covers the same molecule at a fraction of the price, with Phase III Indian trial data behind the approvals. For many patients the real question is not Wegovy vs Ozempic — it is branded vs generic.
