PCOS and GLP-1 Medicines in India: What the Evidence Says
If you have spent years being told to "just lose weight", here is what the 2023 international PCOS guideline and the trials actually say about GLP-1 medicines: what they can help with, what is still uncertain, and what to know about pregnancy.

GLP-1 medicines can help women with PCOS who carry extra weight to lose it, and the 2023 international PCOS guideline says they could be considered alongside lifestyle changes for adults with PCOS and higher weight. The evidence in PCOS itself is still limited: trials show modest weight and BMI loss, with weaker and less certain evidence on periods, testosterone and fertility. They are not for women who are pregnant or trying to conceive, and the guideline asks for reliable contraception while taking them.
Key facts
- In an ICMR-funded study of nearly 9,000 Indian women aged 18 to 40, PCOS affected 7.2% by the strictest criteria and 19.6% by the broader Rotterdam criteria (Ganie et al., JAMA Network Open 2024).
- In the same study, 43.2% of women with PCOS had obesity and 32.9% had fatty liver (full text).
- The 2023 international guideline says GLP-1 medicines "could be considered, in addition to active lifestyle intervention, for the management of higher weight in adults with PCOS" (Teede et al., 2023).
- A 2026 meta-analysis of 11 trials found GLP-1 medicines lowered BMI by about 1.4 points in women with PCOS, with low certainty, and too little evidence to judge effects on periods (European Journal of Endocrinology 2026).
- Semaglutide should be stopped at least 2 months before a planned pregnancy because it stays in the body for a long time (US prescribing information).
Why "just lose weight" was never the full answer
PCOS is not a weight problem with some hormone symptoms attached. For many women it is a metabolic condition, and insulin resistance sits close to its centre. Higher insulin pushes the ovaries to make more androgens, which disturbs periods and drives acne and facial hair, and it makes weight easier to gain and harder to lose. Telling someone in that cycle to eat less and walk more is not wrong, but on its own it is often not enough.
Indian data shows how much is at stake. In the ICMR-funded study, most women with PCOS had at least one metabolic abnormality: 91.9% had abnormal blood fats, 24.9% met the criteria for metabolic syndrome, and 9.1% already had impaired glucose tolerance (Ganie et al., 2024). This is why a PCOS check-up should include blood sugar, HbA1c and a lipid profile, not just an ultrasound. If your sugars are already high, read why your diabetologist should be involved before any weight-loss medicine.
What the 2023 guideline says
The International Evidence-based Guideline for the Assessment and Management of PCOS, updated in 2023 and published in several journals at once, is the reference most PCOS specialists use. On medicines for weight, it says:
- GLP-1 medicines, including liraglutide and semaglutide, could be considered alongside active lifestyle changes for adults with PCOS and higher weight, as for the general population.
- Women taking them should use effective contraception when pregnancy is possible, because safety data in pregnancy are lacking.
- Doses should be increased gradually to reduce stomach side effects.
- Decisions should weigh the possible need for long-term use, since weight often returns after stopping, and long-term safety data are still limited.
- Separately, metformin should be considered for adults with PCOS and a BMI of 25 or more, for weight and metabolic outcomes.
One word in the first line deserves attention: "could". The GLP-1 recommendation is a consensus recommendation, which means experts agreed on it because there was not enough PCOS-specific trial evidence for a stronger one (guideline full text).
What the trials in PCOS show
| Outcome | What trials found | How sure we are |
|---|---|---|
| Weight and BMI | Modest loss: BMI down about 1.4 points across 11 trials | Low certainty |
| Periods | Some trials report more regular cycles, but results vary widely between studies | Uncertain |
| Testosterone | A small fall in total testosterone in pooled trials | Low |
| Natural pregnancy | Higher pregnancy rates in one pooled analysis of small trials | Low |
| Insulin resistance (HOMA-IR) | No clear change in a pooled analysis of placebo-controlled trials | Low |
Most PCOS trials so far are small and short. The large trials of weekly semaglutide and tirzepatide were run in adults with obesity in general and were not designed to measure PCOS outcomes. The 2026 review put it plainly: GLP-1 medicines are associated with modest short-term weight loss in women with PCOS, while the benefits for metabolic, reproductive and psychological outcomes remain uncertain because the data are of low quality. Better trials are needed on periods, skin, hair and fertility.
Pregnancy, fertility and contraception
This is the part that matters most for many women with PCOS, because weight loss can make ovulation more regular and pregnancy more likely, sometimes sooner than expected.
- Planning a pregnancy: the semaglutide label advises stopping at least 2 months before a planned pregnancy, because the medicine stays in the body for weeks (US prescribing information). The tirzepatide label advises stopping when a pregnancy is recognised (US prescribing information). Plan the timing with your doctor.
- On the pill? Tirzepatide can make oral contraceptive pills less reliable. Its label advises switching to a non-oral method, or adding a barrier method, for 4 weeks after starting and for 4 weeks after each dose increase (US prescribing information).
- Trying for a baby with fertility treatment? Weight loss before IVF is a separate conversation with your fertility specialist. Our guide to weight loss before IVF covers it.
Semaglutide or tirzepatide for PCOS?
No trial has compared the two head to head in women with PCOS. In adults with obesity generally, tirzepatide produced more weight loss than semaglutide in the one head-to-head trial, and India's 2025 obesity guideline from the Endocrine Society of India suggests either medicine where a 10 to 15% loss is the goal (ESI guideline 2025). For PCOS, the choice usually comes down to how much weight there is to lose, contraception, side effects and pregnancy plans. Our guide to how doctors choose between the two goes through each factor.
Who should not take a GLP-1 medicine
- Women who are pregnant, or trying to conceive in the coming weeks.
- Anyone with a personal or family history of medullary thyroid cancer, or MEN 2 (a boxed warning on both labels).
- Anyone who has had pancreatitis, unless a doctor has weighed it carefully.
- Teenagers with PCOS, outside specialist paediatric care. ALTRcare treats adults only.
A history of an eating disorder also needs an honest conversation first, because medicines that cut appetite can make some patterns worse.
It was never about willpower
A doctor looks at the whole picture, including insulin, cycles and your plans for a family, and tells you honestly what will help. It starts with a 2-minute eligibility check.
Frequently asked questions
Do GLP-1 medicines help PCOS?
They help women with PCOS and extra weight to lose weight, and the 2023 international PCOS guideline says they could be considered alongside lifestyle changes. Evidence on periods, testosterone and fertility is weaker and less certain so far, because PCOS-specific trials have been small and short.
Can semaglutide make my periods regular?
Some women find their cycles become more regular as they lose weight, and some small trials report this, but a 2026 meta-analysis found too little evidence to be sure. Weight loss itself can restore ovulation, so contraception matters if you are not planning a pregnancy.
Can I take a GLP-1 medicine if I want to get pregnant?
Not while trying to conceive. The semaglutide label advises stopping at least 2 months before a planned pregnancy, and the tirzepatide label advises stopping when pregnancy is recognised. Plan the timing with your doctor.
Is metformin or a GLP-1 better for PCOS?
They do different jobs. The 2023 guideline says metformin should be considered for adults with PCOS and a BMI of 25 or more for weight and metabolic outcomes, while GLP-1 medicines could be considered for higher weight alongside lifestyle changes. Some women take both. Your doctor decides based on your weight, sugars and plans.
Does tirzepatide affect the contraceptive pill?
Yes. Its label advises switching to a non-oral contraceptive, or adding a barrier method, for 4 weeks after starting and for 4 weeks after each dose increase, because it can make the pill less reliable.
